The accident occurred just after 4.20pm on John Renshaw Drive, between the M1 Pacific Motorway and Black Hill Road.
When they arrived, police and NSW Ambulance Paramedics found a 23-year-old woman had died.
She was the driver and sole occupant of a Holden Commodore.
After being assessed by NSW Ambulance Paramedics at the scene, the female driver of a station wagon was taken to John Hunter Hospital. She received further treatment for suspected leg and internal injuries
A male, who was also travelling in the station wagon, sustained minor injuries
The occupants of the third vehicle only sustained minor injuries.
John Renshaw Drive is closed in both directions and motorists have been advised to expect heavy traffic and significant delays.
Westbound motorists are being diverted on to Weakleys Drive and the New England Highway.
Motorists travelling to Black Hill can use Lenaghans Drive and Black Hill Road.
The New England Highway or Hunter Expressway may be used as an alternative route.
Emergency services and the Roads and Maritime Service and the Crash Investigation Unit are on the scene.

Emergency crews are working to free two people who are trapped in vehicles after a crash at Beresfield on Thursday afternoon.
The crash occurred at the intersection of John Renshaw Drive and the M1 Pacific Motorway just after 4.30pm.
The road is blocked in both directions while crews work to free the trapped people.
Motorists are being urged to avoid the area.
