Thompson, who came on in the 57th minute at Cooks Square Park on Sunday, ran onto a volleyed ball across goals from Sean Pratt to score inside to the left post from close range in the 92nd minute.
Thompson and Trott shared heroes' honours with Thompson delivering the goal and goalkeeper Trott keeping Magic at bay with an incredible display guarding the net.
"Magic probably had the better of the match, but the group has been resilient all year and found a way to get there," Magpies coach Mick Bolch said.
"They chased and chased and busted their guts for 98 minutes, but this team is never dead.
"We've scored 14 of our 40 goals in the last five minutes of games. And again today we scored in the 92nd minute, there's belief, there is real belief in the group.
"We don't talk it up a great deal here, but what they've done today is outstanding. From where we were two years ago it is an incredible effort by the squad and all the club.
"Matty Thompson hasn't had much football over the last two months but he come on and was that energetic for us today and to get that back-stick run in the 92nd minute just shows what the whole group is about.
"I couldn't be prouder of them."
Magic had gone ahead in the tie, which was 2-2 after the first leg, in the 85th minute via substitute John Majurovski.
Majurovski couldn't control the ball when it fell to him close to goal but he made no mistake just after the ensuing corner.
James Virgili jumped on the scraps down the right side and found Majurovski at the near post for the crucial touch.
Thompson's leveller sets up a grand final clash with Edgeworth at Magic Park next Sunday. The Eagles beat Lambton Jaffas 1-0 on Saturday and 5-1 on aggregate.
Maitland were under siege for the opening half an hour, but Magic, with the breeze at their backs, could not beat Magpies keeper Matt Trott.
The former Mariners shot-stopper was at full stretch to save a deflected strike in the eighth minute, at his left post to blocked a James Virgili attempt in the 19th and diving low to his right to knock clear a Mitch Oxborrow free kick in the 24th.
Maitland eventually created chances of their own but Magic keeper Paul Bitz was up to the task, blocking a shot from James Thompson in the 36th minute.
The Magpies had their best chance in the 52nd minute when Mat Swan was one on one with Bitz but the gloveman smothered the shot.
Both sides were wide of goals or straight at the keeper with strikes.
Frustrations, and fouls, increased late as both sides searched for the winner. Tempers boiled over briefly with push and shove in the 78th minute, but it was Maitland who had the final say in a high-intensity clash.
