The 34-year-old limped off in the 79th minute of Sunday's 2-0 preliminary final win over Broadmeadow at Magic Park, just after Ryan Clarke put the Magpies ahead 1-0.
The tenacious midfielder hurt his groin in the 2-1 win over Edgeworth in round 12 and missed the following week's 1-0 victory over Valentine.
He made a much-needed return for the Magpies in the elimination semi-final against Azzurri which they won on penalties despite losing Clarke in the warm-up and Liam Thornton in the opening minutes to injury. Swan's twin brother, Andrew, and Matt Comerford were already out for the season with major knee damage.
Mat managed his injury through the 4-0 win over Olympic in week two of the finals but he aggravated it and also hurt his hamstring in last Sunday's success and he was racing the clock to be fit for the grand final.
"It's not the best, but I'm doing everything possible to get there," Swan said on Tuesday. "Originally it was my groin but now it's hammy and groin. Hopefully it's just tightness, but it really hasn't got better since Sunday.
"But I'll just do everything I can because I've obviously missed so many grand finals.
"And there's not too many more opportunities, to be honest. I'm going around one more year and then that's it. It's just getting too hard with family and things like that."
Swan missed Maitland's 2-0 grand final loss to Edgeworth last year because of a European holiday with his girlfriend that he mistakenly booked to start before the decider.
In 2015, the electrician missed Hamilton Olympic's grand final loss to the Eagles because of a job at a mine in Western Australia that started the week of the game.
He and Andrew were travelling overseas when Hamilton lost the 2012 grand final to Magic.
Mat played in Valentine Phoenix's defeat to Hamilton in 2009 but missed Azzurri's loss to Broadmeadow a year earlier because of shoulder surgery.
Swan had an ice bath on Monday, was having physio on Tuesday and was using a hyperbaric chamber on Wednesday as he tried to shake off his injuries.
"It's been a battle and I can't train during the week," he said of the groin issue.
"I've just got to look after it and the week's rest normally gets me through the game."
Swan has been part of a brilliant run for the Magpies, who have won six in a row to make the decider.
"It's just a good group of lads and we all work for each other," he said. "We didn't have much luck at the start of the year but it's starting to come back and we are all very confident."
