NSW Governor David Hurley presented the city's longest serving mayor with the accolade during a ceremony on Wednesday.
Cr Blackmore received the award for service to local government and to the community of Maitland.
He was among around 40 recipients at the ceremony who were recognised for their contributions.
Cr Blackmore attended the ceremony with his wife Robyn and his family. He said the accolade made him reflect on his public life and it was an emotional moment.
He said the award also belonged to his wife Robyn, his family and the people of Maitland who had all been part of his long-standing political career.
“I’ve received so many congratulatory messages from people, but for what? I’m still Peter Blackmore, nothing different,” Cr Blackmore said.
